PREMIERE SURGICAL generally sees new patients in consultation or by referral only.

Please make sure that you bring your insurance cards, a list of your medications, and any films or discs that your referring physician may have given to you.

If you have an appointment already scheduled with us, you can download and print the following forms. Fill them out and bring them with you to your appointment.

INSURANCE

  
PREMIERE SURGICAL participates in many local insurance plans, as well as Medicare and Medicaid. We will need to make copies of all of your insurance cards for our files. So please bring them with you to your appointment. If you change your address, phone number, employer and/or insurance coverage, please notify our receptionist.

If you are a member of an HMO or PPO, please check to see that your Premiere Surgical physician is listed as a provider. Be sure to bring a referral slip from your primary care physician, if one is required by your plan.

We accept Medicare assignments and will file your claim and paperwork. For office visits, you will be required to pay your deductible and co-payment at the time of service. You may pay by cash or personal check.

If you do not see your insurance company listed below, please call our office or your insurance company to verify coverage.
